化脓性脊椎炎的MRI诊断

MRI diagnosis of the pyogenic spondylitis

目的:分析化脓性脊椎炎(PS)的MRI表现特征,以提高其诊断的准确性。方法:搜集经脊椎穿刺活检或临床随访证实的PS共16例,分析其MRI表现。结果:病变多位于腰椎(12蛐16),相邻两个以上椎体受累11例,椎间盘受累10例,椎旁软组织受累4例,硬膜外脓肿6例。16例共累及27个椎体,T1WI呈低信号(25蛐27),T2WI呈略高信号(17蛐27)或高信号(10蛐27)。所有受累椎体形态均无改变。结论:PS的MRI表现具有一定特征,结合临床可以做出明确诊断。

Objective： To analyze MRI features of pyogenic spondylitis（PS） and improve the diagnostic accuracy. Materials and Methods： The MRI appearances of 16 patients with PS were evaluated retrospectively. Results： MRI findings were： most of the PS located in the lumbar spine （12/16）; contiguous vertebrae involvement in 11 cases, involvement of the intervertebral disc, paravertebral soft tissues and epidural abscesses were seen in 10 cases, 4 cases and 6 cases respectively. A total of 27 vertebrae were involved, hypointense on TtWI was observed in 25 vertebrae, slightly hyperintense and hyperintense on T2WI were observed in 17 and 10 vertebrae respectively. Conclusion： There are certain characteristic MRI appearances of PS, usually correct diagnosis can be made by combination of MRI findings with clinical symptoms.
