摘要
目的采用一种改良术式进行犬自体睾丸移植,探讨血管吻合方法对植睾功能的影响。方法切取带主干血管瓣的右侧睾丸动、静脉与同侧髂外动、静脉端-侧吻合进行犬自体睾丸移植。观察移植犬生存情况,对移植睾丸进行影像学检查并比较实验组与对照组睾丸组织学改变及术后不同时间点的性激素水平。结果移植成功率为92·8%(13/14)。术后8周两组犬的睾丸重量、精曲小管直径、间质细胞数及细胞核直径之间无统计学差异(P>0·05)。与术前比较,两组术后各时间点LH明显升高(P<0·05),T及FSH变化无统计学意义;组间比较术后各时间点的T、FSH及LH水平无显著性差异(P>0·05)。彩色多普勒超声及DSA检查提示移植睾丸血供良好。结论改良的自体睾丸移植术可提高移植成功率,且对移植睾丸生精及内分泌功能无明显影响,可进一步应用于异体睾丸移植的实验及临床研究。
Objective To perform testicular autotransplantations in dogs using a modified operation, and study the effect of various vessel anastomosis on the function of the autografis. Methods 20 mongrel dogs were randomly divided into experimental and control groups. After henricastration, right spermatlc artery with a cuff of abdomial aorta ann right spermauc vein with a cuff of inferior vena cava were detached, and tesficular autotransplantation was performed by the end - to - side anastomosis of the spermatic vessels to the external iliac vessels in exprerimental group. In control group, only left testis was removed. The transplant testes were examined by DSA and color Doppler ultrasound, The histological changes of testes were observed, and the serum levels of sexual hormones were determined in both groups. Results The successful rate of testieular autotransplatation was 92.8%( 13/14). There was no significant difference in testicular weights, the diameters of seminiferous tubules, and the number and nuclear diameter of Leydig' s cells between the two groups 8 weeks after operation( P 〉 0.05). Compared with preoperation, serum LH levels were significantly higher after operation in both groups( P 〈 0.05), however the ,serum levels of FSH and T had not obvious changes. There was no significant difference in the postoperative levels of ,serum FSH, LH and T at the various time points between the two groups( P 〉 0.05). DSA and color Doppler ultrasound examination showed that autografts had good blood supply. Conclusion The modified testicular autotransplantation operation could improve the successful rate of transplantation, and had not obvious influence on testicular function. It can be further applied in homologous testicular transplantation experimental and clinical research.
