人工晶状体眼硅油入前房继发青光眼的预防与处理

Prophylaxis and treatment on glaucoma secondary to silicone oil migration into anterior chamber of pseudophakic eyes

目的探讨人工晶状体眼硅油入前房继发青光眼的预防与处理方法。方法对人工晶状体眼视网膜脱离患者12例,在行玻璃体切割硅油填充术后,有硅油入前房继发青光眼者,行Healon注入前房、硅油排出,常规作6点位虹膜周边切除术。结果12眼眼压恢复正常,人工晶状体位正,硅油及视网膜在位,未见不良并发症。结论采用Healon注入、硅油排出联合6点位虹膜周边切除术是治疗人工晶状体眼硅油入前房继发青光眼的有效方法,应采取积极措施预防该并发症的发生。

Objective To study the prophylaxis and treatment on glaucoma secondary to silicone oil migration into anterior chamber of pseudophakic eyes. Methods Following vitrectomy and silicone oil injection, silicone oil migrated into anterior chamber in 12 pseudophakic. Healon is instilled into the eye to facilitate silicone oil aspiration through the another corneal incision,a periphery iridectomy is performed at 6 o＇ clock position routinely. Results Postoperatively,the intraocular lens lies in good position, silicone oil in vitreous cavity and retina remain attached. There is no other severe complications. Conclusion Healon injection into anterior chamber, removal of silicone oil from it with a peripheral iridectomy is a simple, safe and effective procedure dealing with pseudophakic eyes of glaucoma secondary from silicone oil entering into anterior chamber. It is suggested that taking active prophylaxis measures could decrease the rate of this complication.
