原发性肠系膜后恶性肿瘤的诊断与治疗(附39例临床分析)

Diagnosis and Treatment of 39 Cases of Malignant Primary Retroperitoneal Tumor

目的总结原发性肠系膜后恶性肿瘤的诊治经验。方法回顾性分析39例原发性腹膜后恶性肿瘤病人的临床表现、影像学检查特点及手术处理。结果初始临床表现多为腹部肿物,辅助检查CT诊断率为94.4%,肿瘤完全切除率为48.7%,完全切除组术后复发11例,再手术10例,6例完全切除肿瘤。结论发现腹部无症状性肿物要考虑本病,CT是最重要的诊断性辅助检查,手术全切除及整块切除是治疗本病的重要手段,对复发性肿瘤应争取再手术治疗。

Objeetive To summarize the diagnosis and treatment experience in malignant primary retroperitoneal tumor （MPRT）. Metheds 41 patients with MPRT were admitted and treated. Clinical manifestations, imaging findings and surgical management were analyzed retrospectively. Results Abdominal mass was the main symptom and sign. The diagnostic rate by CT was 94.4 %. The whole resection rate was 48.8 %, 11 cases with whole resection had recurrence and 6 eases had resection again.Conclusion Abdominal mass in pstients without other symptoms should be considered MPRTCT seen might be the important method for diagnosis of MPRT. The total resection and en bloc resection could be the best way of treatment. The recurrence tumor shoed be diagnosed early and be treated with reoperation.
