脑梗死患者脑循环动力学改变和川芎嗪对急性缺血性脑损伤的保护作用 被引量:15

Changes of cerebrovascular dynamic indices and protective effect of tetramethylpyrazine on ischemic cerebral damage in patients with acute infarction

摘要 目的:观察急性脑梗死(ACI)患者脑循环动力学改变及川芎嗪对急性缺血性脑损伤的保护作用。方法:选取发病时间在72h以内的颈内动脉系统ACI患者185例,每日静脉滴注川芎嗪注射液200ml,疗程14d,于治疗后不同时间点分别观察脑循环动力学参数(CVDI)、血压、心率、心电图的变化。结果:ACI患者经川芎嗪注射液治疗后CVDI均有明显变化(P<0.05或P<0.01)。与用药前比较治疗后第7d相对平均血流量、平均血流速度、最大血流速度、最小血流速度和压差分别增加45.39%、23.33%、44.64%、185.20%和97.12%,脑血管周围阻力、特性阻抗、波传导速度、动态阻力、临界压力分别降低了55.34%、23.12%、30.93%、22.91%和49.19%,尤以最小血流速度和脑血管外周阻力改变显著,差异具有极显著意义(P均<0.01)。结论:ACI可引起脑循环动力学异常改变,川芎嗪对ACI导致的脑缺血等损伤有保护作用。 Objective: To observe the changes of cerebrovascular dynamic indices (CVDI) in patients with acute cerebral infarction and the protective effect of tetramethylpyrazine on ischemic cerebral damage.Methods: One hundred and eighty -five patients with acute cerebral infarction (ACI) in internal carotid artery system within 72 hours from the oneset of symptoms were given 200 ml tetramethylpyrazine by intravenous drip every day for 14 days. CVDI, blood pressure (BP), heart rate (HR) and electrocardiogram (ECG) were examined at various time points during treatment. Results: CVDI changed significantly after tetramethylpyrazine treatment in patients with ACI (P〈0. 05 or P〈0.01). On the 7 th day after treatment, relative mean volume of blood flow (Qmean), mean velocity of blood flow (Vmean), maximum veloctiy of blood flow(Vmax), minimum velocity of blood flow (Vmin), pressure difference (DP) increased by 45.39 %, 23.33%,44.64%, 185.20% and 97.12% respectively compared with those before treatment, while cerebrovascular resistance(Rv), cerebral vasculor Z (Zcv), wave velocity(Wv), dynamic resistance(DR) and critical pressure(CP) decreased by 55.34%, 23.12%, 30.93%, 22.91% and 49.19% respectively. The changes of Vmin and Rv were most obvious (all P 〈 0.01 ). Conclusion: ACI could induce significant changes of CVDI.Tetramethylpyrazine has protective effect on ischemic brain damage following ACI.
