三磷酸腺苷对窦房结和房室结的影响与迷走神经张力的相关性研究 被引量:3

摘要 目的评价三磷酸腺苷对窦房结和房室结的影响与迷走神经张力的相关性。方法选择18例行电生理检查和导管射频消融术的病人,分别给予阿托品前、后,测量用药前后A-A间期的最大延长量(ΔA-Amax),用S1S1400ms起搏HRA时,依次静脉应用ATP0.05、0.10、0.15、0.20、0.25、0.30mg/kg,直至出现房室传导阻断;然后经静脉给予阿托品0.04mg/kg,用相同的给药方法,测定和观察ΔA-Amax及阻断房室传导所用ATP剂量。结果用阿托品前后,阿托品0.10mg/kg组ΔA-Amax分别为43±9.25ms和26±7.24ms,两者有显著差异(P<0.05);0.20mg/kg阿托品组ΔA-Amax分别为71±12.27ms和37±13.34ms,两者有显著差异(P<0.01)。S1S1400ms起搏HRA时,应用阿托品前,阻断房室结前传所需ATP剂量为0.178±1.02mg/kg,应用阿托品之后,阻断房室结前传所需ATP剂量为0.24±0.98mg/kg,两者之间有显著性差异(P<0.05)。结论腺苷的心脏电生理作用可能存在多种机制:①受体机制,腺苷与心脏腺苷A1受体结合,使膜电位阈值升高,动作电位峰值降低和时程缩短,产生负性变时、变传导作用;②非受体机制,腺苷拟迷走神经作用,使窦房结自律性降低,房室结传导延缓;③其他机制。
