摘要
目的探讨胰腺损伤的诊断、手术方式、术后处理及并发症的防治.方法对16例胰腺损伤的临床资料进行回顾性分析.结果术前确诊3例,术前确诊率仅占16.7%;余均经剖腹手术后确诊.死亡1例,形成假性胰腺囊肿1例,形成胰瘘2例.结论血尿淀粉酶测定对胰腺损伤的诊断无特异性,术前诊断困难,多需术中探查确诊.根据胰腺损伤的部位及程度选择合理的手术方式可降低其并发症和死亡率.
Objective To explore the diagnosis, operational mode, postoperative treatment of and the prevention of its complications. Methods Retrospective analyses of clinical date of 16 cases of pancreatic injury were made. Results Pancreatic injury was confirmed preoperatively in 3 cases （16.7%）. Confirmations were got in others through laparotomy. One ease died. Pancreatic pseudocyst occurred in one case. Pancreatic fistula occurred in 2 cases. Conclusion No specificity was found in hematuria amylase examination of pancreatic injuries. Intraoperative confirmation is till necessary for accurate diagnosis. Feasible operations based on the location of pancreatic injuries and the degree of the disease can effectively reduce its complications and death rates.
