摘要
目的探讨视网膜色素变性(retinitispigmentosa,RP)患者多焦视网膜电图(multifocalelectroretinogram,mf-ERG)特征。方法对临床确诊的RP患者21例42只眼和正常对照组36例50只眼进行了mf-ERG检测。分别对两组中5个环的N1、P1波振幅密度和潜伏期后极部P1波总振幅值和总潜伏期进行比较分析。结果RP患者mf-ERG从中央到周边5个环的P1、N1波振幅密度与正常对照组比较均有下降,差异有非常显著性意义(P<0.01),其中黄斑区(第1环)呈降低反应,黄斑外区(第2~5环)呈较平坦反应。P1、N1波潜伏期与正常对照组比较均延长,但与第1环潜伏期比较,差异无显著性(P>0.05),第2~5环差异有非常显著性意义(P<0.01)。P1波后极部总振幅值与正常对照组比较差异有非常显著性意义(P<0.01)。结论RP的mf-ERG表现为整个后极部视网膜功能的损害,各个区域不尽相同,以周边病变早而严重。mf-ERG检查具有局部定位和定量功能,能发现RP患者的黄斑功能异常。
OBJECTIVE To evaluate the characteristics in patients with retinitis pigmentosa (RP) by multifocal electroretinogram (mf- ERG) .METHODS Data of mf- ERGs were obtained in twenty-one patients (42 eyes) with clinical RP which were diagnosed and in 36 normal volunteers (50 eyes) . Amplitude intensities and latencies of N1-wave and P1 - wave at all 5 rings, general amplitudes and general latencies of P1-wave in posterior retina between two groups were calculated and compared. RKSULTS Compared with the control eyes, amplitude intensities of P1-wave and N1- wave from ring 1 to ring 5 in the patients with RP were totally reduced and there were significant differences between 2 groups (P〈0.01) .In maculae region (ring 1) they recorded the declined responses and out of which(ring 2-5) they showed flat responses. Latencies of P1 -wave and N1-wave from ring 1 to ring 5 in RP were totally higher in the patients with than in the control eyes. In ring 2-5 there were significant differences between 2 groups (P〈0.01), but in ring 1 there were no significant differences (P〉0.05) .There were significant differences in general amplitudes and general latencies of P1-wave in posterior retina between 2 groups (P〈0.01) . CONCLUSIONS Represents of mf-ERG showed that the whole posterior retinal function is damaged in patients with RP, which is different in different area and it is rapid and sever in periphery, mf-ERG could measure locally and quantificationally and find the abnormal macular function.
