早产儿呼吸机相关性肺炎69例临床分析 被引量:2

摘要 目的探讨早产儿呼吸机相关性肺炎(VAP)的临床及病原菌特点。方法69例机械通气时间≥48h符合VAP诊断标准的新生儿作为VAP感染组,与同期机械通气而未感染患儿(对照组)进行分析。结果(1)69例VAP患儿占同期机械通气新生儿的46.6%。(2)上机时间长、胎龄≤32周、体重≤1500g的早产儿易发生VAP;应用大剂量免疫球蛋白可降低VAP发生率。(3)VAP病原菌以铜绿假单孢菌、肺炎克雷伯菌、大肠杆菌为常见;血浆凝固酶阴性葡萄球菌及霉菌感染有上升趋势。结论早产儿呼吸机相关性肺炎(VAP)的发生与多因素有关,对早产儿VAP应采取综合措施进行防治。
