摘要
目的:探讨细胞外基质的变化与喉癌组织恶性度及发生局部淋巴结转移之间的关系。方法:采用免疫组织化学方法检测Ⅳ型胶原、层粘连蛋白、纤维连接蛋白在喉癌中的表达。结果:3者在喉癌中的表达具有一致性;在高分化喉癌中表达多完整,在低分化肿瘤中缺失率明显增加;且随肿瘤浸润程度的加重,3者表达的缺失也逐渐加重,经统计学检验,差异显著(P<0.05);同时在淋巴结转移组与未转移组之间,3种蛋白表达程度亦呈现显著性差异(P<0.01)。结论:细胞外基质中的基底膜与喉癌的细胞分化程度及浸润能力密切相关;活检病理中的基底膜表达可以作为预测喉癌是否发生颈淋巴结转移的参考指标。
Objective: To investigate the relationship between the changes of extracellular matrix and the malignancy degree and local lymph node metastasis in laryngeal squamous cell carcinoma. Methods:The expressions of type Ⅳ collagen, laminin, and fibronectin were detected by immunohistochemical methods. Results:The expressions of these 3 glycoproteins in laryngeal carcinomas were in consistant way. In well differentiated carcinomas, the expression was largely integrated (56%, 44% ,and 36%, respectively). In poorly differentiated carcinomas, the losing rate of expression increased markedly (83.3%, 87.5% ,and 91.7% ,respectively). With the progression of invasion, the losing rate of these 3 glycoproteins increased( P 〈 0.05). There was significant difference in the expressions of 3 glycoproteins between the groups with and without lymph node metastasis( P 〈 0.01 ). Conclusion:Tile basement membrane in the extracellular matrix is closely associated with the differentiation and invasion of laryngeal carcinomas. The expression of basement membrane may indicate the lymph node metastasis in laryngeal carcinomas.
