手术治疗重症溃疡性结肠炎

Surgical Treatment of Severe Ulcerative Colitis

为探索应用全大肠、全结肠切除术治疗重症溃疡性结肠炎的疗效及其生活质量，5例采用全大肠切除、腹部造口术，15例采用全结肠切除、回直肠吻合术。全部病人术后体质迅速改善，各项检验指标大都在3个月内达到正常，全大肠切除者近期无复发，全结肠切除者有5例近期发生直肠溃疡复发。结果显示，重症溃疡性结肠炎的手术治疗是解决其体质衰竭及恶变倾向的最终途径。

The objective of this study was to evaluate the therapeutic effect and life quality of the patients with severe ulcerative colitis after receiving total proctocolectomy and total colectomy. Five cases underwent total proctocolectomy and abdominal stoma,and 15 cases underwent total colectomy and ileorectal anastomosis. The constitutional status of all the cases recovered rapidly postoperation,and all the test indexes were resumed to normal within 3 monthes,no short-term recurrence was found in patients underwent total proctocolectomy,and recurrence of rectal ulcer was found in 5 cases underwent total colectomy. It is suggested that surgical treatment of severe ulcerative colitis is the final mean in solving constitutional failure and cancerous development.
