中医药治疗溃疡性结肠炎95例临床观察

Clinical Observation of the Therapeutic Effect of Tradtional Materia Medica in the Treatment of Ulcerative Colitis（Analysis of 95 cases）

为观察中医辨证中药内服和保留灌肠治疗UC的疗效，将139例UC患者分为两组，治疗组95例，以健脾益气、清热除湿兼化瘀为基本大法，按脾虚湿盛、脾。肾阳虚、肝脾不和、湿热壅滞四型辨证加减内服中药，合用清热解毒燥湿、涩肠止泻中药煎汤保留灌肠；对照组44例，口服水杨酰偶氮磺胺吡啶、复方新诺明，氢化可的松灌肠。治疗组和对照组总有效率分别为96．9％和63．9％，随访6个月至3年，痊愈病例中复发率分别为14．5％和33．3％，二者差异有统计学意义（P〈0．005）。结果表明，口服中药从整体出发，扶正以治本为主；局部中药灌肠，迅速改善病损黏膜，消除或减轻症状体征，祛邪以治标为主。二者合用效果可靠，复发率低，安全易行。

The objective of this study was to observe and evaluate the therapeutic effect of decoction of traditional Materia Medica applied in oral intake and in retention enema in the treatment of ulcerative colitis（UC）. One hundred and thirty nine patients with UC were divided into 2 groups ,patients in group A （95 cases ）received treatment of traditional Materia Medica decoction for oral intake,the main method according to tonicity spleen supplementing qi, clearing away heat removing dampness and removing blood stasis,selection of treatment according to deficiency of spleen and excessive damp,yang deficiency of both spleen and kidney, discord of liver and spleen, stagnancy of damp-heat 4 type, decoction for retention enema,having the effect of clearing away heat, removing toxic substances,eliminating dampness, astringing intestine and relieving diarrhea, while patients in group B（44 cases ）received salicylazosulfapyridine and compound sulfamethoxazole for oral intake and hydroprednisone for enema. As results, the general effective rate of group A was 96.9% ,while that of group B was 63.9% ;and follow up of 6-36 monthes noticed the recurrence rate in group A being 14.5%, that in group B being 33.3%, the difference had statistical significance （ P 〈0. 005）. It is suggested that the combined application of traditional Materia Medica for oral intake according to the whole body codition supporting vital qi and treating primary cause of disease was main mean,and for retention enema was ripid improving mucosa lesions,eliminating or reducing the symptoms and signs. It has the strong points of reliable therapeutic effect, lower recurrence rate and safe medication.
