己酮可可碱对梗阻性黄疸大鼠肝肾的保护作用

PROTECTIVE EFFECT OF PENTOXIFYLLINE ON LIVER AND KIDNEY IN RAT MODEL OF OBSTRUCTIVE JAUNDICE

摘要 ①目的探讨梗阻性黄疸时己酮可可碱(PTX)对大鼠肝、肾保护作用。②方法77只大鼠随机分为3组:假手术(Sham)组7只,胆总管结扎(BDL)组35只,抗肿瘤坏死因子α(TNFα)(PTX)组35只。BDL组与PTX组均作胆总管双重结扎复制梗阻性黄疸模型,两组大鼠随机各分为5个小组,每小组7只。PTX组于术后第1天开始注射5g/LPTX,每100mg体质量1mL,每日1次,直至按计划处死大鼠当日。各小组大鼠分别于术后第1、4、7、10、14天处死,经股动脉采血测总胆红素(TB)、谷丙转氨酶(ALT)、肌酐(Cr)、TNFα,将两组第14天处死大鼠的肝、肾组织进行病理学检查。③结果术后第14天BDL组血清中TNFα的水平明显高于Sham组及PTX组(F=6.33,q=3.18、4.97,P<0.01)。BDL组TNFα与ALT的变化呈正相关(r=0.758,P<0.01),与Cr的变化呈正相关(r=0.502,P<0.05)。PTX组TNFα与ALT的变化呈正相关(r=0.835,P<0.01),与Cr的变化无相关性(r=0.098,P>0.05)。光镜下检查:BDL组及PTX组均可见肝细胞坏死及增生,汇管区可见胆管扩张、胆汁淤积,肝坏死程度以BDL组为重。两组肾脏肾小球无明显改变,肾小管上皮细胞肿胀。④结论梗阻性黄疸时TNFα明显升高。注射PTX对梗阻性黄疸大鼠肝脏的损伤有一定的保护作用;对大鼠的肾脏损伤无保护作用。 Objective To study the protective effects of pentoxifylline on liver and kidney of obstructive-jaundice model in rat. Methods Seventy-seven rats were randomly divided into three groups: sham-operated group (n=7) ; common bile duct ligation group(BDL group, n=35); anti-TNF-αgroup (PTX group, n=35). The model of obstructive jaundice was established in BDL group and PTX group. The rats in the PTX group received 5 g/L pentoxifylline 1 mL/100 mg per rat intraperitoneally every day after operation. Both groups were randomly divided into five sub-groups, with seven rats in each sub-group. Serum ALT,TNF-α, Cr, and TB were determined at 1st, 4th, 7th, 10th, and 14th day respectively after operation. The kidney and liver were observed pathologically at 14th day after operation in both BDL group and PTX group. Results Serum TNF-α in BDL group at 14th day after operation was significantly higher than that in the sham-operated and PTX groups (F=6.33, q=3.18, 4.97, P〈0.01). Serum ALT showed positive correlation with TNF-α in BDL group(r=0. 758, P〈0.01), serum Cr correlated positively with TNF-α in BDL group(r=0. 502, P〈0.05). Serum ALT and TNF-α were well correlated (r=0. 835, P〈0.01) and serum Cr and TNF-α were not correlated in PTX group. Microscopic examination showed that liver cell necrosis was more in BDL group than in PTX group. Bile was filled up in the bile duck in both groups. Renal tubular epithelium was swelling. Conclusion Serum TNF-α rises in rats with obstructive jaundice. Pentoxifylline given intraperitoneally can protect the function of the liver but not of the kidney in rat model of obstructive jaundice.
