膝关节镜术中关节腔内灌注冰冻盐水的护理研究

Study on the nursing for patients with keen joint endoscopic operation combined by joint cavity freezing brine perfusion

目的探讨冰冻盐水灌注膝关节腔行关节镜手术中病人出血量、疼痛、功能恢复等的影响.方法将400例病人分成两组,每组200例,A组冰冻盐水(4～8℃)组,B组常温盐水组.分别于手术时灌注,观察病人的生命体征、术野清晰度和出血量,术后疼痛反映和关节功能恢复情况,两组进行比较.结果A组术野冲洗液红细胞计数比B组少134.5个/L;术后关节内抽液也比B组少10.96 ml,术后关节疼痛时间比B组少30.5 h;下床活动时间比B组缩短22.19 h,住院时间比B组少4 d,手术时间治疗组比对照组少1.01 h.结论4～8℃冰冻盐水灌注关节腔行膝关节镜手术中组织创面出血少,视野清晰,术后病人疼痛反应较轻,膝关节功能恢复快.

Objective To study the effect on amount of bleeding, pain and function recovery for patients with keen joint endoscopic operation combined by joint cavity freezing brine perfusion. Method 400 patients were randomly divided into group A and group B, 200 patients for each group. Group A was given freezing brine perfusion, group B was given room temperature brine perfusion. The patient＇s vital sign, amount of bleeding, post-operative reaction of pain and recovery of joint function etc were observed and compared between the two groups. Result The RBC account of washing liquid for operation field in group A was 134.5 unit/L less than that of group B. The time of joint pain after operation was 30.5 hr less compared with group B. The off bed exercise time was 22. 19 hr less compared with group B. compared with group B, the time for hospitalization was 4 days less, time for operation was 1. 01 hr less. Conclusion 4-8 degree centigrade freezing brine perfusion will be effective in reducing bleeding, pain and enhance joint function recovery for patients with keen joint endoscopic operation.
