摘要
目的讨论DCS和股骨髁解剖钢板治疗股骨髁部骨折的选择、应用效果和功能锻炼。方法收治股骨髁部骨折和髁上骨折38例,双侧2例,均行切开复位,DCS钢板内固定术25膝,股骨髁解剖钢板内固定术15膝,术后早期在CPM上进行被动和非负重主动功能锻炼。结果本组随访36例38膝,时间6-42个月,28例32膝术后功能恢复优良,总优良率84%。结论股骨髁部骨折两髁完整者较适用于DCS钢板内固定,两髁粉碎不完整者较适用于股骨髁解剖钢板,术后早期要进行正规的主、被动功能锻炼。
Objective This paper discussed the selection and apphcations of the treatmg methods on fracture of femoral condyle with Dynamic condylar screw (DCS) and anatomic plate and functional exercises. Methods 38 cases with fracture of femoral condyle and fracture of distal femur were diagnosed and treated, two cases of two sides, open reduction, 25 knee were treated by DCS and 15 knee were treated by anatomic plate, Passive exercises and unload exercises on CPM. Results 36 cases of 38 knee were followed up for 6-42 months, 28 cases of 32 knee had carried out good functional rehabilitations of the femoral condyle, total excellent and good rate was 84%. Conclusion Internal fixation with DCS is applicable to treating intact fracture of two femurs and internal fixation with anatomic plate issuitable for treating shatter fracture of two femurs.
