南方汉族人群白血病患者与HLA基因相关性的研究 被引量:7

摘要 目的研究三种白血病患者与人类白细胞抗原HLA-A、B、DRB1基因的遗传相关性,研究分析对三种类型白血病有易感作用和保护作用的HLA基因。方法以2116例骨髓库的造血干细胞健康捐献者作为正常对照组,124例白血病患者分为慢性粒细胞性白血病(简称慢粒)、急性淋巴细胞性白血病(简称急淋)、急性非淋巴细胞性白血病(简称急非淋)三个病人组,分别比较病人组及正常对照组的HLA-A、B、DRB1位点的基因频率,通过X2检验、显著性差异及疾病的危险率(RR值)的计算,找出病人组与HLA的相关性。结果49例急淋患者中,HLA-A11、B18、B39的RR值分别为1.894、4.0502、2.558;37例急非淋患者中,HLA-B*4001+(B60)RR值为0.0842,明显低于1,HLA-DR9的RR值为2.3165;38例慢粒患者中,HLA-B54、DR4的RR值分别为2.9865、1.9251,HLA-A11的RR值为0.3069,明显低于1。结论研究表明HLA-A11、B18、B39基因与急淋相关联,有易感作用;HLA-B60、HLA-DR9与急非淋相关联,HLA-DR9有易感作用,HLA-B60有保护作用;研究还发现HLA-A11、B54、DR4与慢粒相关联,HLA-B54、DR4有易感作用,而HLA-A11有保护作用。 Objective To study the correlation of three types of leukemia patient and HLA - A,B, DRB1 gene in southern Chinese Han population and to investigate the HLA gene with susceptible role and protective role. Methods 2116 bone marrow registry donors were used as normal control group, 129 leukemia patients were divided into three groups:acute lymphoblastic leukemia(ALL) group, acute non-lymphoblastic leukemia (ANLL) group, chronic granulocytic leukemia(CML) group. The HLA-A, B, DRB1 gene frequencies of three leukemia patient groups were compared with the normal control group respectively. The X^2 test and relative risk (RR) value were calculated for each HLA antigen so as to reveal the susceptible or protective HLA gene in each patient group. Results Among forty-nine ALL patients, relative risk (RR) for HLA-A 11, B18, B39 are 1.894, 4.0502, 2.558 respectively. Among thirty-seven ANLL patients,RR value for HLA-B*4001, DR9 are 0.0842, 2.3165 respectively. Among thirty-eight CML patients, RR value for HLA-B54, DR4, A11 are 2.9865,1.9251, 0.3069 respectively. Conclusions Our results indicate that HLA-A11, B18, B39 gene are highly correlated with ALL and contribute the susceptible role. HLA-B60, DR9 are highly correlated with ANLL, HLA-DR9 contribute the susceptible role, However, HLA-B60 contribute the protective role. HLA-A11, B54, DR4 are highly correlated with CML, HLA-B54, DR4 contribute the susceptible role and HLA-A11 contribute the protective role.
