摘要 目的研究鼻咽癌survivin基因表达和血管内皮生长因子(VEGF)的表达及其临床意义.方法运用逆转录聚合酶链反应技术(RT-PCR)检测鼻咽癌组织中survivin、VEGF mRNA的表达情况.结果35例鼻咽癌组织中23例survivin表达阳性(65.7%),22例VEGF表达阳性(62.9%);14例鼻咽黏膜慢性炎症组织中2例survivin表达阳性(14.3%),3例VEGF表达阳性(21.4%).癌组织与炎症组织相比,survivin、VEGF两者均有表达,但具有显著性差别(P<0.05).鼻咽癌组织中survivin的表达与肿瘤的细胞分化及TNM分期有关,高分化者的表达率明显低于中低分化者(P<0.05),Ⅰ~Ⅱ期的阳性表达率(47.4%)较Ⅲ~Ⅳ期(87.5%)低(P<0.05).鼻咽癌组织中VEGF的表达与颈淋巴结转移及TNM分期有关,有颈淋巴结转移者其阳性表达率较高,Ⅰ~Ⅱ期的阳性表达率(42.1%)较Ⅲ~Ⅳ期(87.5%)低(P<0.01).鼻咽癌组织中survivin、VEGF的表达呈正相关性.结论鼻咽癌组织中存在survivin、VEGF的过表达现象.survivin、VEGF基因的异常表达与鼻咽癌的发生发展存在一定的关系.survivin的异常表达可能在鼻咽癌的血管生成中发挥一定的作用. Objective This study was designed to investigate the expression and clinical significance of survivin gene and VEGF in nasopharyngeal carcinoma(NPC). Methods Reverse transcriptase polymerase chain reaction(RT-PCR) was used to detect survivin and VEGF mRNA in NPC tissues. Results The positive expression of survivin gene and VEGFmRNA in NPC tissues is about 65.7% (23/35) and 62.9% (22/35) ,compared to chronic nasopharynxitis of about 14.3% (2/14) and 21.4 % (3/14) ,which is statistically significant(P 〈0.05). Survivin positive expression less frequently appeared in well differentiation of NPC, compared to mediate/poor ones(P〈0.05). It also showed stronger positive expression in stage Ⅲ-Ⅳ of NPC than that in stage Ⅰ-Ⅱ(P〈0.05). VEGF positive expression in cases of NPC with cervical lymph node metastasis was higher than that without metastasis(P〈0.05). And VEGF showed stronger positive expression in stage Ⅲ-Ⅳ of NPC than that in stage Ⅰ-Ⅱ(P〈0.01 ). Conclusion Their expression may be linked with oncogenesis and development of NPC. The abnormal expression of survivin may play a part in angiogenesis progression of NPC. There was positive correlation found between survivin and VEGF expression in NPC.
