北京市城市社区老年人伤害疾病负担的研究

An epidemiological study on disease burden of injury among the elderly dweller in Beijing city community

目的探讨北京市城市社区老年人伤害发生现状及其疾病负担。方法以居委会为单位,采取随机整群抽样的方法,调查北京市牛街地区60岁及以上老年人在其过去一年中的受伤情况及伤后影响等。结果被调查者在过去一年中共有311例、420人次伤害发生,涉及12种伤害。伤害总发生率(人)为13.55%,人次发生率为18.30%。因伤死亡1例。伤害的危害涉及个人、家庭、社会三个层面,包括躯体、精神、经济损失等多方面。跌倒不仅发生率最高,伤情最严重,而且经济损失最大。结论北京市城市社区老年人群伤害发生与死亡水平位居中等水平;跌倒已经成为威胁该人群生存与健康的重要公共卫生问题。

Objective To investigate the disease burden of injury among elderly dweller in community of Beijing city. Methods A random cluster sampling was used to collect information. Results There were 311 persons experienced 420 times of injuries during the past years among 2 295 subjectives aged 60 and above. Total incidence of injury was 13.55%, and disability rate was 87.14 per 100 000. The leding three kinds of injuries were： falls, injuryied by sharp/obtuse articals. Total medical cost was 102 647RMB per year, while the highest one being falls. The disease burden of injuries included three respects：individual, family, and society. Injuries were harmful to the elderly＇s physiologically and psychologically. Conclusions Incidence of injury and injury death among the elderly dweller in community of Beijing city was at medium level. Falls had been one of the important public health problems.
