彩色多普勒在人工血管移植术后随访观察中的应用(附21例报告)

摘要 目的探讨彩色多普勒对人工血管移植术后应用的价值。方法对人工血管移植术后21例患者的26条移植血管进行彩色多普勒检查,观察移植血管腔内及周围的二维超声表现,测量血管各段的内径及吻合口大小,应用彩色多普勒血流显像检测移植血管内的血流通畅情况及最大流速。结果21例患者移植的26条人工血管中,术后1个月及1个月以上保持血流通畅20条,占76%;吻合口狭窄3条,占9%,其中近侧端狭窄1条,远侧端狭窄2条;术后1个月移植血管内未探测到血流信号及血流频谱(血栓形成)2条,其中1条合并周围脓肿形成,后行截肢术;术后3个月移植血管内未探测到血流信号及血流频谱1条。结论彩色多普勒可应用于对人工血管移植术后疗效的观察,可作为长期的随访工具。 Objective To explore the application value of color Doppler flowing imaging (CDFI) for posttransplantation of artificial vasa. Methods Totally 26 artificial vasa transplanted on 21 eases were examined with 2D-CDFI. The inner and surrounding of the artificial vasa were observed with 2I)-ultrasound, the internal diameter of the artificial vasa and the anastomotic stoma were measured, and the signal and rate of blood flow in the artificial vasa were detected with CDFI. Results Among 26 artificial vasa of 21 cases, the blood flow keeping unobstructed was detected in 20 (76%) vasa one month and more than one month after operation; the stricture of anastomotic stoma was detected in three (9%) vasa, including one proximal and two distal; no blood flow signal and frequency spectrum were detected in two vasa one month after operation, one of which had concurrent peripheral abscess-formation one week after operation, and afterward this case was amputated~ no blood flow signal and frequency spectrum were detected only in one vas three months after operation. Conclusion CDFI could be applied to observe the posttransplantation curative effect of artificial vascular and served as a tool for long-term follow-up.
