摘要
目的探讨2型糖尿病血浆纤溶酶原激活物抑制物(PAIΙ)及其基因启动子区4G/5G多态性及其胰岛素抵抗的关系。方法对近2年来我院诊断为2型糖尿病的住院患者156例,通过检测血浆纤溶酶原激活物抑制物(PAIΙ),并进行PCR扩增PAI1基因启动子区4G/5G基因多态,同时测定血糖水平、糖化血红蛋白(GhbA1)、空腹血胰岛素、血浆胆固醇、甘油三脂,并与有关因素作相关分析研究PAI1与胰岛素抵抗的关系。结果(1)T2DM组与正常组相比,PAI1基因型频率分布有明显的统计学差异,等位基因频率无明显统计学意义。(2)2型糖尿病组4G/4G组PAI1水平显著高于其他两组。(3)对照组中3组基因型的PAI1水平、TCH、TG、LDLC、HDLC、空腹胰岛素、HOMA差异均无显著性。(4)T2DM组3组基因型的PAI1水平、TCH、TG、LDLC、HDLC、FBG、空腹胰岛素、HOMA均明显高于对照组中相应基因型者。在4G/4G组,PAI1水平与BMI、TG、空腹胰岛素、WC、HOMA都有关。结论在2型糖尿病中,PAI1水平与PAI1基因启动子区基因型有关;PAI1水平与胰岛素抵抗有关,且PAI1水平和甘油三酯和空腹胰岛素的关系与PAI1基因启动子区基因型有关。
To investigate the relationship of circulating plasminogen activator inhibitor-1 antigen levels and genotype at a single nucleotide insertion/deletion(4G/5G) polymorphism in the promoter region of PAI-1 gene with insulin resistance in T2DM. Methods 156 in-patients with type 2 diabetes as defined by WHO criteria, and 60 normal control were analyzed. 4G/5G allele polymorphism in PMI-1 gene promoter region were genetyped by polymerase chain reaction amplification of genomic DNA by using allele specific primers. Meanwhile, blood sugar, GHbAI, fasting insulin, TCH,TG, etc. were measured and the association of PAI-1 with insulin resistance was analyzed. Results The frequency of PAI-1 4G/SG genotype showed significant difference between T2DM and normal control group. PAI-1 in 4G/4G genotype was significantly higher than that in the other two genotypes. In three genotypes of T2DM, PAI-1, cholesterol, triglyceride,fasting glucose, fasting insulin, HOMA were significantly higher than that in the corresponding genotypes of normal control group. The correlation between PAI-1 and BMI, TG, fasting insulin, WC,HOMA existed only in 4G/4G genotype. Conclusion This study indicates that in T2DM, PAI-1 level is associated with insulin resistance and genotype in the region of PAI-1 gene promoter. The association between PAI-1 level and both triglyceride and insulin is influenced by genotype in the region of PAI-1 gene promoter.
