以听力障碍为首发症状鼻咽癌的临床分析

Clinical Analysis of Early Hearing Loss in Nasopharyngeal Carcinoma

目的探讨以听力障碍为首发症状鼻咽癌的诊断和治疗。方法回顾10例以听力障碍为首发症状的鼻咽癌患者的临床资料。10例患者均经听力检测,同时进行间接鼻咽镜或纤维鼻咽镜、CT和新生物的病理学检查。确诊后接受7～8周的放疗,随访听力恢复情况。结果电测听显示传导性耳聋9例,混合性耳聋1例;鼻咽镜或纤维鼻咽镜检查发现局部有明显新生物病理学检查证实10例患者中,鼻咽癌Ⅰ期、Ⅳ期各2例,Ⅱ期、Ⅲ期各3例。放疗后1月,5例Ⅰ期和Ⅱ期患者的听力明显改善结论对于主诉听力障碍的患者,应常规检查鼻咽部。以听力障碍为首发症状鼻咽癌的早期诊断和治疗,对于患者的听力恢复至关重要。

Methods ： Objective To explore the diagnosis and treatment of early hearing loss in nasopharyngeal carcinoma. The clinical data of 10 patients with early hearing loss in nasopharyngeal carcinoma were analyzed retrospectively. The patients underwent hearing test, indirect laryngoscopy or fiberoptic pharyngorhinoscopy, CT, and neoplasm pathologic observation. The diagnosed patients undertook 7-8-week radiotherapy and were followed up. Results Nine of 10 patients had conductive deafness and 1 mixed deafness. Obvious neoplasm was observed under indirect laryngoscopy or fiberoptic pharyngorhinoscopy. Pathologic observation confirmed 2 patients in Ⅰ, Ⅳ stage and 3 in Ⅱ ,Ⅲ stage of nasopharyngeal carcinoma, respectively. The auditory functiort of 5 patients were improved obviously after 1 month radiotherapy. Conclusion Routine examination of nasopharynx is given to the patients with hearing disturbance as the chief complaint. It is important that timely diagnosis and treatment should be given to the patients with early hearing loss in nasopharyngeal carcinoma for their auditory function recovery.
