急性颅脑损伤的BAEP与颅内压、CT的关系

The Relationships of Clinic,CT,ICP and BAEP in Acute Head Injury

通过61例急性颅脑损伤患者的研究,发现脑外伤后,BAEP的异常率与GCS负相关,损伤越重,颅内压(ICP)越高,BAEP异常越明显。同时表明,CT显示中线移位越重,侧室、环池受压越显著,BAEP异常率越高。提示颅脑损伤时,颅内压、CT及BAEP呈同步变化。BAEP结合CT检查可以互补,说明神经生理学检查技术BAEP与神经结构学变化CT及生物物理学指标ICP间有内在关系。强调BAEP对脑损伤病情判断有重要价值。

The correlations between clinic.CT,ICP and BAEP have been studied in 61 patients with acute head in-jury. We found that abnormal BAEP changed more obviously in the deterioration of patient's condition and raised ICP; and had a negative correlation with GCS (P<0. 05). When ICP≥4kPa(30mmHg) ,the abnormal-ity of BAEP changed mofe obviously compared to controls(.P<0. 05). BAEP abnormality also reflects colsed relationship with the CT feature. When the midline shift from 5mm to more than 10mm on CT scan.the rate of abnormal BAEP increased up to 100% from 61. 5%. And there is,a postive correlation with the compres-sion of the ambient cisten(the rate of abnormal BAEP from 14. 29% to 100% ,P<0. 05) or lateral ventricles (the rate of abnormal BAEP up to 85. 71% from 12. 5% ,P<0. 05). Therefore,combined exarninations of CT scanning and BAEP are helpful in evaluating the changes in anatomical structures and neurological functions of the injured brain. The prolongation or lack of wave v lateney of BAEP in severe head injury was the featare of primary brainstem injury. At the same time.BAEP is also valuable for guiding the treatment and useful in predicting the prognosis.
