内窥镜下Müller′s动作观察对悬雍垂腭咽成形术适应证选择的意义

Predicative value of müller's maneuver in OSAS patients related to UPPP

对１６例因阻塞性睡眠呼吸暂停综合征拟行悬雍垂腭咽成形术（ＵＰＰＰ）者，术前在纤维内窥镜下行Ｍüｌｌｅｒ′ｓ动作观察。结果是：以软腭水平狭窄或阻塞为主者１１例（甲组），以喉咽水平狭窄或阻塞为主者５例（乙组）。ＵＰＰＰ后甲组１１例中８例获得较好疗效，有效率７２．７％；乙组５例中仅１例有效，有效率２０％。两组间疗效差别显著。提示术前行Ｍüｌｌｅｒ′ｓ动作观察，了解咽腔阻塞或狭窄水平，可作为临床选择ＵＰＰＰ适应证、提高ＵＰＰＰ疗效的有效方法。

Abstract Fibred endoscopic observation with the muller's maneuver(FEMM)was applied in 16 patients with obstructive sleep apnea syndrome (OSAS). There were 11 patients with OSAS whose most predominant pharyngeal collapses were in the soft palate level, and 5 patients in hypopharyngeal. All patients were treated by uvulopalatopharyngoplasty(UPPP),and operative satisfactary results were fairly good in 8 patients of the oropharyngeal collapse group,but only one in the hypohparyngeal collapse group.The results reveal that FEMM is a useful method in selection for UPPP candidates.
