心脏复跳后输入不同液体对肺超微结构的影响

摘要 本文用18只杂交犬随机分为三组,均在戊巴比妥钠30mg/kg肌注麻醉下气管插管,行控制呼吸。右侧进胸,作主动脉阻断缺血停跳2min后开放。观察了心脏复跳后输入三种不同液体对肺超微结构的影响。Ⅰ组(LR组)用平衡液20ml/kg以2ml/kg/min输入;Ⅱ组(LRBD组)以1/2平衡液及1/2全血+地塞米松0.5mg/kg按上述量输入;Ⅲ组(HTS组)以10ml/kg之7.5%氯化钠输入。结果CVP、MAP以LR组上升至基线时间最长,HTS组最快,LRBD组介于两者之间。LPO三组均有上升,LRBD组迅速下降有显著保护作用。在光镜及电镜下显示LR组有明显肺间质水肿及Ⅱ型细胞部分损害,LRBD组间质水肿不明显,Ⅱ型细胞亦较完整。故从本实验初步结果,认为以LRBD组之混合液体输入·较好。 he influence of fluid infusions on pulmonary ultrastructure after heart resuscitation was studied in dogs. Eighteen dogs were randomly devided into 3 groups in group LR Ringer,solution,20ml/ks(2ml/kg/min)was insused, in group LRBD,1/2Ringer’s solution + 1/2 whole blood + 0.5mg/kg of dexamasone(same amount andspeed as group LR),and in group HTS,7.5%sodium chloride(10mg/kg),The results show that after heart resus-citation,the time for CVP and MAP to restore the baseline in group LR was the longest and in group HTS theshortest。 The values of LPO in all 3 groups were increased markedly during cardiac arrest.LPO in group LPBDwas decreased rapidly after resuscitation indicating its protective effects。 There were remarkable pulmonary inter-stitial edema and damage to type Ⅱ cells in group LR,slighter tissuse damage in group HTS and very slight damagein group LRBD,It is concluded that the mixed injection of LRBD has an obvious advantage in protecting the pul-monary ultrastructure from cardiac arrest.
