洛赛克、泰胃美与钙通道阻滞剂治疗消化性溃疡的近期与远期疗效观察

The Short-term and Long-term Efficacy of Omeprazole、Tagamet and Calcium-channel Bloking Agent in the Treatment of Peptic Ulcer

用洛赛克、泰胃美及钙通道阻滞剂治疗127例经胃镜证实的活动性消化性溃疡。洛赛克组(简称洛组)35例,每晨服20mg,疗程4～6周。泰胃美组(简称泰组)32例,每晚服800mg,疗程为8周。钙通道阻滞剂组(简称CCBA组)60例,服硝苯啶10mg,tid或尼莫地平20mg tid,CCBA组分为CCBA-Ⅰ组(疗程为8周)和CCBA-Ⅱ组(长期用药)。洛组、泰组、CCBA组的溃疡痊率分别为97.1％,87.5％,83.3％。洛组、泰组、CCBA-Ⅰ组的6个月复发率分别为23.5％,32.1％,34.6％,12个月复发率分别为35.3％,71.4％,73.1％。长期服药的CCBA-Ⅱ组6个月内无复发,12个月内仅1例复发。上述结果提示洛赛克是治疗消化性溃疡非常有效的药物,而钙通道阻滞剂长期服用能有效地预防溃疡复发。

One hundred and twenty-seven patients endoscopically documented active peptic ulcer were divided into four groups. 35 patients (omeprazole group) were treated with omeprazole 20 mg in the morning for 4-6 weeks. 32 patients (tagamet group) were treated with tagamet 800 mg at night for 8 weeks. 60 patients (CCBA group) were treated with nifedipine 10 mg tid or nimodipine 20 mg tid. 31 patients (CCBA-I group) of them were treated for 8 weeks and 29 patients (CCBA-K group) of them were treated for long-term. The age, sex and size of ulcer between the groups were comparable. After a course of treatment the peptic ulcer healing rate of the omeprazole tagamet and CCBA group was 97. 1%, 87. 5% and 83. 3% respectively. The recurrence rate of the omeprazole, tagamet and CCBA-I group was 23. 5%, 32. 1% and 34. 6% in 6 months after the cessation of medicine and was 35. 3%, 71. 4%,73. 1% in 12 months after the cessation of medicine respectively- In the CCBA-Ⅱgroup. No one was reurrent in 6 months and only one patient was recurrent in 12 months. In conclusion-Omeprazole is very effective drug for the treatment of peptic ulcer, and it is effective prevention that calcium-channel blocking agent is used for long term.
