摘要
本文采用超声心动图测定了无心功能不全症状的高血压病患者右室结构和功能,并将其分为左室肥厚(LVH)组和非左室肥厚(NLVH)组.结果两组病人右室收缩功能和室腔大小均正常.NLVH组与正常组比较通过三尖瓣的E峰较低,A峰较高,E/A比值较小,快速充盈分数(RFF)、1/3充盈分数(1/3FF)和每搏量校正的最大充盈率(PFR)降低,下降时间延长;LVH组A峰、E/A比值、PFR进一步损害,右室壁厚度较正常组和NLVH组明显增加.表明高血压病患者早期右室舒张功能已受损害,但收缩功能正常,左室肥厚的同时存在右室肥厚.
To examine right ventricular(RV) function and structure in essential hypertension,echocardiography studies were performed in 64 asymptomic essential hypertensive patients——30with normal left ventricle (NLVH) and 34 with echocardiographic evidence of left ventricular hypertrophy (LVH). 32 age- and sex-matched normotensive subjects acted as control group. The results indicated RV systolic function and sizes were normal in all hypertensive patients, while RV early peak velocity (E) was lower in NLVH;atrial peak velocity (A) was higher; rapid filling fraction (RFF) ,1/3filling fraction(1/3FF) and peak filling rate corrected to stroke volume (PFR)were lower ; deceleration time was prolonged. In LVH cases atrial peak velocity, ratio of E/A and PFR were further damaged. In comparison with the normotensive subjects and hypertensive patients without LVH, right ventricular wall thickness was increased in the hypertensive patients with LVH. Data indicate that RV diastolic function is abnormal and systemic function is preserved in asymptomic essential hypertension. The patients with LVH also have a thicker right ventricular wall.
