胎儿肾动脉与中大脑动脉血流、羊水指数及膀胱容积间关系的超声学研究

摘要 本文采用AlokaSSD680型超声诊断仪,检测8例正常妊娠38—39周胎儿的RA·RI、MCA·RI、BV及AFI值。按膀胱由小到大顺序顺时针调正顺次后,分为三组进行统计学处理。结果:(1)各项的三级间无明显差异;(2)每组的四项值间及各项均值间均无明显相关关系。但随着时间推移,各组值、各项指标均有一定倾向性变化:RA·RI值呈减少倾向时,BV值呈增加倾向,AFI值呈减少倾向。推测正常妊娠晚期胎儿,子宫-肾脏反射已建立;肾动脉与中大脑动脉血流受不同机制调控;胎儿在血流动力学方面、排尿等出现的周期性交替现象可能与胎儿中枢神经系发育有密切关系。 Abstract Ultrasonic diagnostic apparatus of Japanese ALOKA SSD 680 type was used to detect the RA- RI,MCARI,BV and AFl in & normal fetuses of 38-39 geslational weeks. The four mentioned indexes were detected every an hour, for a total of three times. The values were arranged from minimum to maximum according to the bladder volume and divided into threegroups. The statistical result indicated:(1)No significant difference between the three groups for each index.(2)No obvious correlation between the three values,neither the average value of each index. Except for MCA-RI,each index tended to change astime going on. When RA-RI value have tendency of decreasing, BV have tendency of increasing, while AFl value tends to decrease. It suggested that for fetus,in normal late gestation uterine-kidney reflex have been formed, the hemodynamics of RA andMCA is controlled by different mechanisms. There might be close relationship between fetal hemodynamics, periodical alteration ofmicturation and the development of fetal central nerve system.
