大剂量大容积5-氟尿嘧啶腹腔化疗治疗消化系恶性肿瘤

Prospective trial of intraperitoneal chemotherapy of 5-Fu in patients with advanced gastrointestinal cancers

本文对23例中晚期胃肠道恶性肿瘤患者术后采用大剂量大容积5-Fu 腹腔化疗。全组患者共接受42个疗程、225次腹腔化疗,化疗期间无毒性反应,对肝、肾功能无影响。无肠穿孔、出血、腹腔化脓感染和严重肠粘连等严重并发症。药代动力学比较,腹腔液5-Fu 峰值浓度约是周围静脉血浓度的100倍,并维持了持久恒定浓度梯度差。结果表明,腹腔化疗简便易行,无严重毒副作用和并发症,是治疗中晚期胃肠道恶性肿瘤的一种有前途的辅助措施。其最终临床效果有待进一步研究。

23 Patients with advanced gastrointestinal cancers recieved 225 times in 42 courses of intraperitoneal chemotherapy of high doses in large volume of 5-Fu. Hematologic,hepatic and renal toxicities as well as seri- ous complications,such as intestinal perforation,bleed- ing,infection and serious peritoneal adhesions in peri- toneal cavity,were not observed.The peak concentration of 5-Fu in peritoneal fluid was 100 times that in the sys- temic circulation,Results indicated that intraperitoneal chemotherapy was a safe,simple adjuvant treatment route without serious complications for treating advanced gastrointestinal cancers.Further studies are necessary to determine the fired clinical efficacy of intraperitoneal chemotherapy.
