原发性小肠肿瘤20例临床分析

Primary Small Intestmal Tumors :Analysis of 2O Cases

作者报告原发性小肠肿瘤２０例，其中男性１２例，女性８例，年龄４５岁～７９岁，平均６２．１岁。２０例中良性５例，恶性１５例。肿瘤分布在十二指肠、空肠各６例，回肠８例。恶性肿瘤以腺癌最多，其次为平滑肌肉瘤和淋巴系统恶性肿瘤。腹痛、上消化道出血和肠梗阻是本组小肠肿瘤的主要症状，不同程度的贫血和腹块是常见体征。本组临床诊断与手术及病理对照，误诊率为５５．０％。除１例未手术外，余１９例均作了手术治疗。作者就小肠肿瘤的发病情况，小肠肿瘤的临床表现及诊断上的困难，结合本组病例特点进行了讨论，认为治疗小肠肿瘤以手术为主，手术方法按肿瘤的病理性质和所在部位而定。

From January l 9 6 6 to December 1 9 9 2,twenty cases of primary small intestinal tumors weretreated.There were l 2 cases of males and 8 cases of females. patients ranged in age from 4 5 to 7 9 years with anaverage of 62.1.Among them l 5 cases were malignant and 5 cases were benign , The tumor site in duedenum andjejunum was in 6 cases respectively ,and in lieum was in 8 cases. In malignant cases the adenocarcinoma took the firstplace , the leiomyosarcoma and malignant tumors of lymphatic system were i n the second place. Abdomind pain,bleedings of upper gastrointestinal tract ,intestinal obstruction are the main symptoms ,abdominal mass and anemia invarying degrees are the common signs in tumors of small intestine.The misdiagnosis rate in our series was 55%. Allpatients except one underwent operative treatment. The morbidity ,clinical features ,difficulties of dlagnosis and thesurgical management were discussed. It is suggested that operation is the main therapeutical measure for primary smallintestinal tumors and the operative procedures should be determ i ned according to the pathology and location of the tumors.
