肾小球肾炎低补体血症的相关因素

Factors Associated with Hypocomplementemia in Glomerulonephritis

对1540例不同类型肾小球肾炎低补体(C_3、C_4)血症的相关因素进行了临床与病理分析。本组病人低C_3和C_4症的发生率为7.7％和17.5％,不同类型肾小球肾炎的低补血症发生率相差很大。按人数计,狼疮性肾炎、膜增殖性肾炎和内皮系膜性肾炎占低C_3血症患者的64.2％,占低C_4血症患者的78.2％,IgA肾病占原发性肾小球肾炎低补体血症的第一位。低补体血症与肾小球肾炎的病理类型、病程、高丙球蛋白血症、乙肝病毒血症有关,与膜性肾病和IgA肾病的临床表现、肾小球免疫球蛋白沉积有显著相关。

The factors associated wit h hypocornplementemia were studied m 1546 patients with various types of glornerulonephritis. The incidence of depressed serum third(C3)and fourth(C4)components of cornplements were 7. 7% and 17. 5% respectiveiy .varied in different t'ype.s of glomeiulonephritis. Lupus nephritis,membrano-proliferative glomerulonephritis and cndocapillarv niesangial glomeru-lonephritis only accoumed for 64. 2% and 78.2% in patients with depressed serum C3 and C4 lev-els,IgA nephropathy accounted for the largest number in patients with depressed C4 level in prima-ry glomerulonephritis. Hypocornplementemia (C4) were associated with higher serum IgG level in most types of glomerulonephritis, with the course of endocapdlary mesangial glomerulonephritis, with the mcidence of hepatitis B surface antigenemia in membranous nephropathy and membra-noproliferative glomerulonephritis,and with glomerular immune deposits in membranous nephropa thy and IgA nephropathy.
