低位直肠癌根治性切除保肛术式102例临床应用

Clinical Application of the Radical Operation of Low Rectal Cancer with Preservation of the Anus

作者介绍一种经腹低位直肠癌根治性切除，保留肛门、乙状结肠拖下固定术式。9年来共施行手术102例，占同期低位直肠癌（肿瘤下缘距齿线2cm～8cm）手术总数的86．4％（102/118）。术后2年肿瘤复发率为7．7％，术后5年生存率为65.5％。术后8周、12周和半年内完全恢复肛门自控排便功能者分别占86％、99％、100％。肛门功能恢复程度均属优良。作者对此术式的特点，并根据手术疗效对保留肛门功能的合理性及其根治性进行讨论。

The authors introduce a radical operation of low rectal cancer with preservation of the anus, pull through and fix of the sigmoid colon. In nine years, this type of operation has been performed 102 times, which accouted for 86. 4 % (102/118) of low rectal cancers (the low end of the tumor was Zcm to 8cm from the dentate line ). The recurrent rate of postoperation for 2 years is 7. 7 %, the survival rate of postoperation for 5 years is 65. 5%. The rates of complete recovery of self-control function of the anus and excremenlation after 8 weeks, 12 weeks and six months were 86%, 56, 99%. and 100%; respectively. The function of the anus is excellent. The authors also discuss the rationality and the possibility of radical operation with preservation of the anus according to the therapeutic effectiveness of the operation.
