儿童肱骨髁上骨折的旋转移位与分型(附50例X线片分析)

Rotional Malposition and Types for Children's Supracondylar Fracture of the Humeri(with Analysis of 50 X-rays Pictures)

本文通过对50例 X 线片分析,从结果中看出,肱骨髁上骨折远折端在纵轴上旋转移位发生率占有移位骨折中的82.35%。为鉴别旋转移位,介绍了从 X线片上测量远、近两断端横径的宽度和临床上观察肢体体位的方法;对 X 线片上出现两断端横径不相等的机理,旋转移位产生的机制及愈合后的影响,从理论上进行了阐述;同时对分型采取了以远折端移位方向做标准,将该骨折分为伸直类型、屈曲类型、旋转类型三种,这样使远折端向不同方向的所有移位,基本上全部概括了进去,分型的名称也充分体现了各个方向移位的实质问题。

According to analysis of 50 X-rays pictures,this article shows that the rates of rotational malpo- sition of the Supracondylar Fracture of the humeri are 82.35% of all the rotational fractures,which occur at farer end in its vertical axis.For recognizing rotational malposition,it presents a method of measuring the di- ameter of both far and near broken cross sections by examining the X-rays pictures as well as a clinical method of observing the body position of limbs.And also it explains theoretically the mechanism of umequal diameters on both 2 broken cross sections appearing in the X-rays pictures,the causitive mechanism of rota- tional malposition and its influence in healed.Meanwhile,the article takes the direction of rotation at far end as a standard,diviving varied fractures into 3 types:extension,flexion and rotation.Thus it includes basically all kinds of malposition in different directions at far end,the r(?)mes of which fully represent the real charac- teristics of malposition in all directions.
