糖尿病肾血流变化的彩色多普勒超声观察

Observation on color Doppler flow images of renal blood flow in patients with diabetes mellitus

用彩色多普勒血流显象（ＣＤＦＩ）和脉冲多普勒超声心动图（ＰＤＥ）检测２６例肾功能正常的非胰岛素依赖型糖尿病病人及２０例正常人的双侧肾动脉血流，以探讨糖尿病肾病血流动力学改变。糖尿病病人与正常人对照，肾动脉内径增宽（Ｐ＜０．０１），血流量增加（Ｐ＜０．０１），搏动指数及阻力指数降低（Ｐ＜０．０５）。结果表明，糖尿病临床肾病前期呈高灌注、低阻力改变。提示ＣＤＦＩ和ＰＤＥ可为临床早期发现糖尿病肾病提供一项简便易行的非创伤性检测方法。

The blood flow of bilateral renal arteries in 26 non-insulin dependent diabetic patients with normalsenal function and 20 normal individuals were measured by color Doppler flow image (CDFI) and blood flow spectrum of pulse Doppler echoeardiography (PDE) to study the changes of the hemodynanics in diabetic nephropathy. Compared with normal individuals,the internal diameter and the volume flow of renal artery in diabetes were increased (P<0.01). The pulsatility and resistance index showed that in decreased (P<0.05).The results indicated that the early stage of diabetic nephropathy there were high perfusion and low resistance. It is suggested that CDFI and PDE are convenient and non-invasive methods for early diagnosis of diabetic nephropathy.
