应用下腹部横形腹直肌肌皮瓣再造乳房

ABSTRACTRECTUS FEMORIS MYOCUTANEOUS FLAP FOR RECONSTRUCTION OF BREAST

应用下腹部横形腹直肌肌皮瓣进行４例乳房再造，其中２例为皮瓣带蒂转移，于乳癌根治切除术后即刻完成乳房再造；２例为血管吻合肌皮瓣游离移植，分别于根治术后２个月和５年进行乳房再造。术后游离皮瓣完全成活，带蒂皮瓣分别于术后２～３周出现脂肪液化，经换药痊愈。４例乳房外形基本满意。随访１０个月～１８个月，２例游离移植未见异常，１例带蒂转移于术后１年出现腹壁薄弱膨隆，另１例失访。

i Senkai;Xu Jun, et al. (PlasticSurgery Hospital,Beijing 100041.) Rectus femoris muscle of the lower abdomen was used as myocutaneous flap in the recon-struction of the breast in 4 patients.of which , in 2 cases the reconstruction was carried out im-mediately following the radical mastectomy for breast cancer by using trancfer of the pedicled my-ocutaneous flap,and in the other two cases,the vascularized free myocutaneous flap was used 2months and 5 years after the radical mastectomy,tespectively. The vascularized free myocuta-neous flaps were survived,however,in the pedicled myocutaneuus flap group,the fat of the ftapshad liquefaction 2-3 weeks after operation. The latter were healed after repeatea dressings. Theexternal appearance of the 4 reconstructed breasts looked sattisfactory. The patients wete follwedup for 10 to 18 months,2 patients had no ill effects, while the 2 pedicled myocutaneous flaps,inwhich 1 patient had bulging of the weakcned abdominal wall,and the other had lost from followup.
