心电图平板运动试验中一过性电轴左偏的临床意义

CLINICAL SIGNIFICANCE OF TRANSIENT LEFT AXIS DEVIATION DURING TREADMILL EXERCISE TEST

报道７例心电图平板运动试验中出现的一过性电轴左偏，并除外了心率依赖现象。其中仅４例合并有缺血型ＳＴ－Ｔ改变。３例冠状动脉造影证实有左前降支冠状动脉病变，另４例核素心肌显像示前壁缺血。此种情况值得重视。

Seven patients with rate-independent transientleft axis deviation during treadmill exercise test were reported, only 4 of them showed ischaemic ST-T changes. However , coronary arteriography and radionuclide myocardial imaging revealed left anterior descending artery over 75% stenosis in 3 patients, and ischaemia on anterior wall in the other 4 patients ,respectively. So this finding may reflect left anterior fascicular block resulted from ischaemic injury of the left anterior decending coroary artery.
