血必净注射液对急性肺损伤兔炎性介质及内皮功能影响的实验研究

Effects of Xuebijing Injection on Inflammatory Mediators and Endothelial Function during Acute Lung Injury in Rabbits

目的:探讨血必净注射液对急性肺损伤(ALI)兔炎性介质的影响及其内皮功能的保护作用。方法:采用随机分组对照试验方法。取24只新西兰兔随机分为3组,每组8只。空白对照组予静脉注射生理盐水2mL/kg,ALI组予静脉注射内毒素0.5mg/kg,血必净治疗组在静脉注射内毒素0.5mg/kg后给予静脉注射血必净5mL/kg。分别测定3组0h、8h时点的血清肿瘤坏死因子α(TNF-α)、白介素-6(IL-6)水平,并在光镜下观察实验兔的组织形态改变和测定肺组织W/D比值。结果:8h后ALI组肺脏体积增大,光镜下见肺组织内大量白细胞浸润,肺间质水肿明显,肺泡腔及间质可见血性渗出液。ALI组肺组织W/D比值、血清TNF-α、IL-6明显升高。血必净治疗组肺组织W/D比值、血清TNF-α、IL-6亦有升高,但明显低于ALI组(P<0.001)。与ALI组比较,血必净治疗组肺间质、肺泡水肿及肺出血情况明显减轻。结论:血必净注射液具有拮抗炎性介质和保护内皮功能的作用。

Objective: To investigate the effects of Xuebijing injection on inflammatory mediators and endothelial function during acute lung injury(ALI) in rabbits. Methods: 24 New Zealand rabbits were randomly divided into 3 groups: control group,ALI group and treament group. Control group received normal saline 2 ml/kg intravenously.ALJ group received endotoxin 0.5 mg/kg intravenously.Treatment group were treated by intravenous Xuebijing 5 mL·kg-1 after endotoxin 0.5 mg/kg intravenously.The levels of tumor necrosis factor α (TNF-α) and interleukin-6(IL-6) in serum and dry/wet ratio of lung weight(W/D) were detected in three groups at the time of 0 hour (T0h) and after 8 hour (T8h). At the same time lung specimens were obtained to observe the histological changes under a microscope. Results:After 8 hours,ALI group's lung volume increased. Under the microscope, a large number of leukocyte aggregated and infiltrated in the lung. pulmonary edema was obvious, pulmonary alveolar cavity and interstitial bloody exudate.Compared with control group, W/D ratio, serum TNF-α and IL-6 of ALI group increased significantly. After treatment, W/D ratio, serum TNF-α and IL-6 of Treatment group increased, but lower than those of ALI group significantly(P<0.001).Compared with control group, interstitial lung or pulmonary edema and pulmonary hemorrhage decreased significantly (P<0.001). Conclusions: Xuebijing injection has positive effects on decreasing the levels of inflammatory mediators and improving endothelial function.
