新生儿缺血缺氧性脑病血浆脑利钠肽的变化及临床意义

The change and clinical significance of BNP in plasma of the children with newborn HIE

摘要 目的 研究新生儿缺血缺氧性脑病(HIE)患儿血浆脑利钠肽(BNP)水平的变化,探讨血浆BNP水平与新生儿HIE严重程度之间的关系. 方法 采用放射免疫法测定54例HIE患儿血浆BNP水平,并与22例对照组患儿比较.结果 中、重度HIE患儿急性期血浆BNP水平分别为(896±358)、(1043±590) ng/L,明显高于对照组(234±60) ng/L和轻度组(334±38) ng/L(P<0.01);中、重度HIE患儿恢复期血浆BNP水平分别为(346±50)、(432±90) ng/L明显低于急性期水平;但中、重度HIE患儿中7例有神经系统后遗症者在恢复期(14 d)血浆BNP水平为(823±298) ng/L明显高于预后良好的30例(336±42) ng/L(P<0.01),且持续到6个月. 结论 新生儿HIE患儿血浆BNP水平的变化与HIE的严重程度密切相关. Objective To study the change of BNP(Brain natriuretic peptide) in plasma of the children diagnosed with newborn Hypoxic-ischemic encephalopathy (HIE) and discuss the relationship between the level of BNP in plasma and the severity of HIE. Methods Testing the level of BNP of 54 children with HIE by radioimmunoassay (RIA) and comparing to 22 children in control Group. Results The level of BNP of children with moderate HIE in the acute period was (896±358)ng/L and the severe one was (1043±590)ng/L.In the recovery period, the moderate one was(346±50)ng/L and the severe one was (432±90)ng/L, which were obviously lower than the acute level. However, in the recovery period there were 7 cases with nervous system sequelae whose level was (823±298)ng/L, which was much higher than the 30 cases well recovered with the level (336.2±42)(P<0.01),lasting 6 months. Conclusions The level of BNP in plasma of children with newborn HIE is related with the severity of HIE.
