救治重型颅脑损伤早期纤维支气管镜吸痰加肺泡灌洗的临床意义

Clinical research of early fiberbronchoscope sputum combining with alveolu lavage on curing severe traumatic brain injury

目的探讨早期纤维支气管镜吸痰加肺泡灌洗对纠正重型颅脑损伤患者缺氧及防治肺部感染的临床意义。方法分析比较重型颅脑损伤患者早期纤维支气管镜吸痰加肺泡灌洗50例和常规吸痰50例动脉血氧纠正情况,肺部感染发生率,肺部感染控制时间。结果早期纤维支气管镜吸痰加肺泡灌洗能明显迅速改善缺氧,纤维支气管镜吸痰组肺部感染发生率及肺部感染治愈时间显著低于非纤维支气管镜吸痰组(P<0.01)。结论对重型颅脑损伤患者可先考虑早期纤维支气管镜吸痰加肺泡灌洗取代常规吸痰以通畅呼吸道,尤其是合并明显缺氧、肺部感染患者。

Objective To investigate the clinical effect of early fiberbronchoscope sputum combining with alveolus lavage on curing anoxia and pulmonary infection caused by severe traumatic brain injury.Methods 50 cases of traumatic brain injury patients who treated with early fiberbronchoscope sputum combining with alveolus lavage were compared with patient treated with normal sputum, their atery oximeter adjustment,pulmonary infection rate,control time of pulmonary infection were analyzed.Results Early fiberbronchoscope sputum combining with alveolus lavage can obviously improve hypoxia,pulmonary infection rate and infection healing time of fiberbronchoscope sputum group was obviously lower than non-fiberbronchoscope sputum group(P<0.01) .Conclusion As for evere traumatic brain injuried patients,early fiberbronchoscope sputum combining with alveolus lavage maybe firstly considered,especially suit for hypoxia complication and pulmonary infection patients.
