睾丸非霍奇金淋巴瘤11例临床病理观察

Clincal pathological features of testicular non-hodgkin lymphoma

目的探讨睾丸非霍奇金淋巴瘤临床病理特征及免疫表型。方法对11例睾丸非霍奇金淋巴瘤进行临床资料、HE切片及免疫组化观察。结果 11例睾丸非霍奇金淋巴瘤患者年龄49～82岁;临床主要表现为无痛性弥漫性的睾丸肿大,病变部位右侧6例,左侧4例,双侧1例;组织学类型:弥漫性大B细胞淋巴瘤(DLBCL)9例,滤泡型淋巴瘤1例,外周T细胞淋巴瘤(非特殊性)1例。结论中老年睾丸无痛性弥漫性肿大或肿块应首先考虑非霍奇金淋巴瘤,组织学上需要与睾丸精原细胞瘤、转移性低分化癌、肉芽肿性睾丸炎以及恶性黑色素瘤等鉴别,确诊依赖于病理组织学和免疫组织化学检查。

Objective To observe the clinical pathological features,diagnosis,differential diagnosis and immunophenotype of testicular non-Hodgkin lymphoma.Methods 11 cases of testicular non-Hodgkin lymphoma were observed clinically and pathologically with HE and immunohistochemistry.Results 11 cases of testicular non-Hodgkin lymphoma aged from 49 to 82 years.The most common clinical manifestation was a painless scrotal swelling.Testicular lymphoma has a rather high incidence of unilateral involvement and bilateral testis were also often involved.Diffuse large B-cell lymphoma(DLBCL) was the most common histotype in the testicular non-Hodgkin lymphoma.Conclusion The painless testicular swelling of older adults should be consider the testicular non-Hodgkin lymphoma.Differential diagnosis included seminoma,metastatic poorly differentiated carcinoma,granuloma testitis,and melanoma.The final diagnosis of testicular non-Hodgkin lymphoma depend on the histopathology and immunohistochemistry.
