常规心电图呈一度房室阻滞并一侧束支阻滞的动态心电图观察

The observation of holter monitor electrocardiography of patients with frist-degree atrial ventricular block and unilateral bundle branch block on standard 12-lead ECG

目的观察常规心电图一度房室阻滞(AVB)并一侧束支阻滞(BBB)的动态心电图(DCG)变化。方法对29例资料完整者的DCG进行分析。结果 16例常规心电图与DCG诊断无变化;3例除本身心电图特征外,合并极度窦性心动过缓;6例出现二度AVB并一侧BBB;3例出现间歇性左、右束支交替传导;1例出现不全性三支阻滞。结论对年龄较大同时合并有器质性心脏病者,当心电图上出现一度AVB并一侧BBB时,有潜在发生重症心律失常的风险,必须引起高度重视。

Objective To observate the changes of holter monitor electrocardiography of patients with first-degree atrial ventricular block and unilateral bundle branch block on standard 12-lead ECG. Methods The holter monitor electrocardiographys of 29 patients were analysed. Results The diagnosis of standard 12-lead ECG was the same as that of holter monitor electrocardiography for sixteen patients.The holter monitor electrocardiographys of six patients showed.second-degree atrial ventricular block and unilateral bundle branch block.The holter monitor electrocardiographys of three patients showed intermittent right bundle branch block or left bundle branch block.The holter monitor electrocardiography of one patient showed imcomplete three-bundle branch block. Conclusion The old patients with organic heart disease should be paied attention to the risk of serious arrhythmia when the first-degree atrial ventricular block and unilateral bundle branch block was showed on their standard12-lead ECG.
