IVU与MRU在泌尿系统先天畸形疾病中的诊断价值比较

Comparation of intravenous urography and MR urography in the diagnoses of the urinary congenital abnormities

目的:探讨静脉尿路造影(IVU)与磁共振尿路成像(MRU)两种方法对泌尿系统先天畸形疾病的诊断价值。方法:对比分析41例经手术及病理证实的泌尿系统先天畸形患者的IVU及MRU影像学资料。结果:11例肾盂输尿管连接部狭窄病例,IVU显示6例,可明确诊断,显影不良2例及未显影3例,均未明确诊断;MRU全部显影,均明确诊断。9例先天性巨输尿管、10例输尿管口囊肿及2例腔静脉后输尿管病例I,VU及MRU显示清晰,均明确诊断。7例重复肾输尿管I,VU显示5例,明确诊断,2例显影不良,未明确诊断;MRU均明确诊断。2例神经源性膀胱I,VU显示1例,明确诊断,1例显影不良,未明确诊断,MRU均明确诊断。结论I:VU是泌尿系统先天畸形主要检查方法,对IVU不显影或显影不良及做其他检查诊断与鉴别诊断困难时,MRU可作为补充。

Objective: To compare the imaging features between intravenous urography(IVU)and MR urography(MRU) for providing the best modality in the diagnoses of the urinary congenital abnormities.Methods: The IVU and MRU images of 41 patients with urinary congenital abnormities were compared and analyzed.All the cases were pathologically proved.Results: In 11 ureteropelvic junction stenosis cases,6 cases were displayed,2 cases were displayed incompletely,3 cases undisplayed on the IVU,but on the MRU all the stenosis locations were displayed.Displayed 9 congenital megaureters,10 ureteral cysts,2 retrocaval ureters were displayed all completely on both IVU and MRU.In 7 kidney and ureter duplication,5 cases were displayed,2 cases were displayed incompletely on the IVU;but on the MRU all were displayed completely.In 2 neurogenic bladders,1case displayed completelyed,1case displayed incompletely on the IVU;but on the MRU all were displayed completely.Conclusion: IVU is the main technique in the diagnosis of the urinary congenital abnormities,and the MRU is a complementary in case of necessity technique.
