低钾性周期性麻痹患者红细胞膜Na^+-k^+-ATPase活性变化及相关激素研究 被引量:1

摘要 目的研究原发性低钾性周期性麻痹(HOKPP)的发病机理及影响因素。方法采用磷钼酸比色法测定了30例HOKPP患者发作期和恢复期及30例健康人的Na+-k+-ATPase(以下简称NKA),同时应用放免法测定HOKPP发作期和恢复期胰岛素(INS)、皮质醇(F)、生长激素(GH)、睾酮(T)水平。结果HOKPP患者发作期(20.07±5.74umol.pi/106RBC/h)显著高于恢复期NKA(14.33±4.13umol.pi/106RBC/h)和健康对照组NKA(14.59±3.91umol.pi/106RBC/h)(P<0.05);恢复期HOKPP患者与健康对照组NKA无显著差异(P>0.05)。HOKPP患者发作期INS、F、CH显著高于缓解期(P值分别为<0.05和<0.001),两期T水平差异无显著性。以血钾、激素和NKA的关系做直线相关分析,发现发作期和恢复期K与NKA呈负相关,(分别为r=-0.823 P<0.001;r=-0.596 p<0.01)发作期INS与NKA呈正相关(P<0.001r=0.855)。结论①HOKPP发作期血K+降低是NKA活性增加的结果;②发作期的胰岛素水平升高是低钾的主要原因。 Objective To ivesugate the pathogenic mechanism and the efect factors of hypoklemic periodic paralysis(Hokpp).Methods The activities of Na^+-k^+-ATPase(NKA) Was measured by colrimetric method of phosphomolybdic acid in 30cases with HOKPP episode and intermitence,and in 30 healthy persons for control.Results The leve ls of blood insulin(INS)、F、GH、 T measured by A in HOKPP episode were significantly higher than those in intermittentence(P<0.05)and in controls(P<0.05).The levels of blood insulin、F、GH in Hokpp episode weresignificantly higher than those in intermiuentence(P<0.05、P<0.05、P<0.001).Negative correlation were found between the blood K^+ and the activities of NKA in HOKPP episede and intermittentence(r_1=0.823,r_2=0.596).Postitive correlation was also found between the level of insulin and the activities of NKA in HOKPP episede.Conclusion The high activities of HOKPP episode which related the high level of insulin caused the hypokalemia.
