Sideburns Reconstruction with Expanded Scalp Flap

目的总结并介绍利用扩张头皮瓣修复鬓角的方法。方法对鬓角的形状及其相对于周围器官的位置进行界定,从而大致对鬓角定位及勾勒轮廓。以此为基础,对56例鬓角处瘢痕、血管瘤、色素痣患者,利用扩张的头皮瓣进行治疗。对于单侧鬓角缺损者,按照对侧鬓角的形态和大小制作布样,然后在患侧扩张头皮瓣上设计,然后按照移位皮瓣转移修复;对于双侧鬓角缺损的患者,在理论位置上勾勒出再造鬓角的形状让患者选择,然后按选择的形状按单侧缺损的步骤修复。结果本组利用扩张头皮瓣修复鬓角66个,皮瓣全部成活,再造鬓角形状、位置好,双侧基本对称,头发生长方向与正常基本一致。修复效果良好。结论组织扩张术是修复鬓角缺损一项很好的方法,良好的效果来源于恰当的皮瓣设计。

Objective To summary and introduce the method to reconstruct sideburns with expanded scalp flap. Methods According to the rough location and outline of the normal sideburns, the reconstructed sideburen was roughly located and outlined. Base on these knowledge, 56 patients with sideburns defect secondary to scar, hemangioma, pigmented nevus were treated with expanded scalp flaps. As to the unilateral defect, a sample of the sideburns was made from the normal side firstly. Then a flap was designed according to the sample on the expanded scalp. The flap was transposed to reconstruct the affected sideburns. When treating the bilateral sideburns defect, different shape of the sideburns were drown on the theoretical position. The chosen shape was the sample of the sideburns, and it was done following the approach mentioned above. Results In these series, 66 sideburnss were reconstructed with expanded scalp flap. No flap loss appeared. The location and shape of the reconstructed sideburns were good. The symmetry was obtained. The hair pattern was almost normal. The result was satisfactory. Conclusion It was an idea method to reconstruct the sideburns with the expanded scalp flap, good result comes from reasonable flap design.
