摘要
目的探索眶上动脉连续跨区供血的乳突皮瓣修复鼻缺损的可能性。方法多普勒血流仪探明眶上动脉→颞浅动脉→耳后动脉与耳后乳突皮瓣联系无误后,制成以眶上动脉供血的岛状乳突皮瓣,经鼻背至额部的皮下隧道转移至鼻头行鼻头再造。结果1例鼻头缺损患者应用此法再造鼻头获得成功。结论约有57%人群的颞浅动脉在耳廓上极上1cm内有一耳后分支分布到耳后乳突区与耳后动脉吻合,且乳突区皮瓣色泽与面部相近,因此本技术可为鼻再造提供一个新的可选方法。
Objective To investigate the feasibility of reconstruction of nose defect with the flap at the mastoid region vascularized by supraorbital artery. Methods The communication from supraorbital artery to posterior auricular artery through superficial temporal artery was confirmed by Doppler.The island mastoid flap which was vascularized by supraorbital artery was tunnelled to reconstruct nose tip. Results One patients with nose tip defect was successfully treated with this method.Conclusion 57% of the superficial temporal artery has a posterior auricular branch which distributes in the mastoid region and anastomoses with posterior auricular artery.The mastoid flap has a similar color with the face and can be selected for nose reconstruction.
