电子胸腔镜治疗包裹结核性胸膜炎双盲随机对照研究 被引量:4

Treatment of Encapsulated Tuberculous Pleurisy with Medical Thoracoscopy:a Randomized Double-blind Controlled Clinical Trial

摘要 目的:探讨电子胸腔镜对包裹性结核性胸膜炎近期疗效。方法:36例包裹性结核性胸膜炎患者在传统治疗(抗结核+抽胸腔积液+口服小剂量强的松)基础上行胸腔镜手术治疗。一月后复查X线胸片及胸部B超,比较2组胸膜腔粘连程度评分及患者抽取胸腔积液次数,并观察不良反应。结果:胸腔镜改善胸膜粘连的有效率达94.4%,高于传统治疗组(45%),抽胸腔积液次数也明显减少,无严重副反应。结论:胸腔镜治疗结核性胸腔包裹积液安全、有效。 Objective To explore the recent cruative effects of thoracoscopy in treatment of encapsulated tuberculous pleurisy.Methods The thoracoscopy were additionally given in 36 patients with encapsulated tuberculous pleurisy based on the conventional treatment,such as anti-tuberculosis,discharging pleural effusion and oral low-dose prednisone.The pleural adhesion scores,times of dischaging and side effects were compared between two groups by rechecking the X-ray and type-B ultrasonic after one month.Results The amelioration of pleura adhesion in thoracoscopy group(94.4%) was significantly higher than that of conventional treatment group(45%),and the times of dischaging the pleural effusion was significantly reduced,no serious side effects had happened.Conclusion The thoracoscopy could be safety and effectively used in treatment of encapsulated tuberculous pleurisy.
