慢性鼻-鼻窦炎功能性鼻窦内镜手术426例临床治疗分析

Retrospective Analysis of 426 Patients with Chronic Rhinosinusitis after Functional Endoscopic Sinus Surgery

目的分析慢性鼻-鼻窦炎手术无效的原因,提高慢性鼻-鼻窦炎的手术疗效.方法对2005年12月至2010年2月间住院的426例慢性鼻-鼻窦炎(伴或不伴鼻息肉)行功能性鼻窦内窥镜手术,如有伴有鼻中隔偏曲或慢性肥厚性鼻炎的患者同期行鼻中隔成形或下鼻甲及中鼻甲整形术.结果术后定期随访1 a,其中有效例数393,有效率92.2%;无效33例,无效率7.8%.结论做好术前术后的综合治疗,术后鼻腔冲洗及鼻窦内窥镜下换药,术中微创操作有助提高慢性鼻-鼻窦炎的手术效果.

Objective To analyze the causes of failure in patients with chronic rhinosinusitis after functional endoscopic sinus surgery,and to improve the surgical curative efficacy.Methods 426 patients with chronic rhinosinusitis(with or without polyps)undertook the functional endoscopic sinus surgey between December 2005 and Feburary 2010.Nasal septal,middle and inferior turbinate plasty were also operated if patients had a deviated nasal septum or chronic hypertrophic rhinitis.Results After one year follow-up,393 cases were effective and the effective rate was 92.2%,33 cases recurred and the ineffictive rate was 7.8%.Conclusion To improve the efficacy,we must emphasize:comprehensive therapy,postoperative nasal irrigation,sinonasal dressing,minimally invasive operation.
