宫颈癌盆腔淋巴结转移的术前CT诊断价值 被引量:10

The Value of Preoperative CT Scan in Diagnosis of Pelvic Lymph Node Metastasis of Cervical Carcinoma

摘要 目的探讨螺旋CT在宫颈癌盆腔淋巴结转移术前诊断中的价值.方法 50例经临床和宫颈活检诊断为宫颈癌Ⅰa~Ⅱa期的患者,宫颈癌根治术前1周行上、中、下腹和盆腔螺旋CT检查,与术后病检为金标准对照,计算CT检查的灵敏度、特异度,制作ROC曲线.结果 CT诊断宫颈癌淋巴结转移的敏感度和特异度分别为78.9%和90.3%,所制作的ROC曲线下面积是0.845.结论 CT是宫颈癌淋巴结转移较为理想的影像学方法,进一步提高术前诊断率,将对年轻的早期宫颈癌患者的卵巢保护有重要意义. Objective To investigate the value of preoperative CT scan in detecting pelvic lymph node metastasis of cervical cancer.Methods 50 patients,who were diagnosed as cervical cancer in Ⅰa-Ⅱa stage by clinical and cervical biopsy,received CT scan one week before surgery.The imaging and pathologic findings for each patient were compared.The sensitivity and specificity of CT in predicting metastasis lymph nodes were analyzed and the ROC curve was drawn.Results The sensitivity of CT in diagnosis of pelvic lymph node metastases of cervical cancer was 78.9%,and the specificity was 90.3%,and the area of the ROC was 0.845.Conclusions CT is an ideal iconography method to diagnose lymph node metastasis of cervical cancer.Improving the preoperative diagnosis rate has an important value in the ovary protection of young patients with cervical carcinoma in early stage.
