舒芬太尼麻醉用于心血管手术的多中心临床研究

Sufentanil anesthesia for cardiovascular surgery:a randomized multicentre clinical trial in China

目的评价舒芬太尼用于心血管手术麻醉的有效性和安全性.方法北京阜外心血管病医院等11所医院参加该项中心研究.选择全身麻醉、低温、心肺转流(CPB)下行冠状动脉搭桥术和/或瓣膜置换术的病人共400例,随机分为芬太尼(F)组(100例)和舒芬太尼(S)组(300例).详细观察并记录麻醉诱导、术中、术后病人的血液动力学指标,术后病人清醒时间,气管拔管时间,与舒芬太尼或芬太尼相关的其他不良反应以及伴随的特殊治疗.结果两组病人术后恢复良好.血液动力学参数、术后清醒时间、拔管时间无显著性差异.两组病人未见明显不良反应.结论舒芬太尼与芬太尼的临床效价比接近于5～7:1.两者均可安全应用于心血管手术麻醉.

Objective To evaluate the validity and safety of sufentanil anesthesia used in cardiovascular surgery. Methods Four hundred patients undergoing valvular surgery and/or coronary artery bypass grafting （CABG） under cardiopulmonary bypass （CPB） from 11 hospitals in China were enrolled and randomly divided into fentanly group （ 100 cases） and sufentanil group （300 cases）. The changes of hemodynamics during induction,operation and postoperation, recovery time and extubation time,and the side effects associated with fentanyl or sufentanil and special treatments were observed.Results All patients in both groups recovered well without any marked side effects. There were no significant differences in hemodynamics, recovery and extubation time between two groups. Conclusion The efficacy of sufentanil is five to seven times that of fentanyl. Both sufentanil and fentanyl can be used safely for cardiovascular surgery.
