硅油填充眼复发视网膜脱离的再手术治疗

Management of recurrent retinal detachment in silicone Oil-filled eyes

目的 探讨硅油填充眼复发视网膜脱离的原因、手术方式选择及临床疗效。方法 对24例(24只眼)硅油填充术后复发视网膜脱离的患者再手术眼进行回顾性分析。结果 9只眼选择视网膜冷凝联合扣带术，7只眼视网膜复位；15只眼选择硅油取出、剥膜、再次眼内填充术，其中视网膜切开达4个象限2只眼，3个象限1只眼，2个象限2只眼，局部松解性切开3只眼，10只眼视网膜复位。结论 增生膜的形成是硅油填充眼复发视网膜脱离的主要原因，但只要根据视网膜裂孔及增生膜情况选择适当的手术方式，仍可取得较为理想的疗效。

Objective To explore causes of failure,surgical methods options,and outcomes after reoperations for recurrent retinal detachment in silicone oil-filled eyes. Methods 24 patients were admitted because of recurrent retinal detachment in silicone oil-filled eyes. The prognostic outcomes were reviewed retrospectively. Resuits 9 silicone oil-filled eyes with recurrent retinal detachment were managed with scleral buckling,retinal detachment in 7 eyes. 15 silicone oil-filled eyes with recurrent retinal detachment were managed with revision of vitrectomy with membrane surgery with or without retinectomes, retinal detachment in 10 eyes. Conclusion Reoperation is effective for recurrent retinal detachment in silicone oil-filled eyes .
