胃癌及癌旁组织中Bcl-2和Ki-67蛋白表达及其意义

EXPRESSION AND SIGNIFICANCE OF Bcl-2 AND Ki-67 PROTEIN IN GASTRIC CARCINOMA AND ITS PARATISSUE

①目的探讨胃癌及癌旁组织中Bcl-2和Ki-67蛋白表达的意义及其相关关系.②方法采用S-P免疫组织化学染色法检测58例胃癌及癌旁组织Bcl-2和Ki-67蛋白的表达.③结果胃癌、伴癌前病变和不伴癌前病变癌旁黏膜组织中Bcl-2阳性表达率分别为44.8%(26/58)、68.8%(11/16)和21.4%(9/42),胃癌及伴癌前病变癌旁黏膜组织中Bcl-2蛋白的表达明显高于不伴癌前病变癌旁黏膜组织(x2=12.139,P<.05).但其表达与胃癌的分化程度、淋巴结转移、病理学分期及浸润程度无关(x2=0.129～3.652,P>0.05).胃癌、伴癌前病变和不伴癌前病变癌旁黏膜组织中Ki-67平均指数分别为38.6±15.0、34.3±10.3和12.4±10.9,胃癌、伴癌前病变癌旁黏膜组织中Ki-67蛋白的表达明显高于不伴癌前病变癌旁黏膜组织(F=50.80,q=14.0、8.1,P<.01),Ki-67蛋白在Bcl-2阳性胃癌组织中的表达明显高于Bcl-2阴性胃癌(t=2.07,P<.05).④结论Bcl-2与癌变前期过程有关,其抑制凋亡是胃癌发生的早期事件.Ki-67表达对胃癌的进展程度评估有一定意义.

Objective To study the expression and significance of Bcl-2 protein and Ki 67 protein in gastric carcinoma and the tissue by its side. Methods Tissues from 58 gastric carcinomas and their sides were tested for the expression of Bcl-2 protein and Ki-67 protein by S-P immunohistochemical method. Results The Bcl-2 immunoreactivity was detected in 26（44.8%） of 58 gastric carcinomas（GC）, 11 （68. 8% ） of 16 paracarcinomas with precancerous lesions（PLs） and 9 （ 21. 4% ） of 42 paracarcinomas without PLs. The expression of Bcl-2 in GC and paracarcinomas with PLs was more than in paracarcinomas without PLs, the difference was significant （χ^2=12. 139,P〈0.05）. No significant difference statistically was found in pathological differentiation, lymph node metastasis or clinical stages （χ^2=0.129-3. 650, P〉0.05）. The Ki-67 index was 38.6~15.0 in gastric carcinomas, 34.3±10.3 in paracarcinomas with PLs and 12.42：10.9 in paracarcinomas without PLs. The Ki-67 index in GC and paracarcinomas with PLs was higher than in paracarcinomas without PLs, the difference being significant （F=50. 80, q=14.0,8. 1; P〈 0.01） The expression of Ki-67 protein in Bcl-2-positive gastric carcinomas was higher than in Bcl-2-negative gastric carcinomas （t=2.07, P〈0.05）. Conclusion The expression of Bcl-2 protein is associated with the early stages of PLs, which is an early event in which Bcl-2 protein prevents cells apoptosis in gastric carcinomas. The expression of Ki-67 protein is associated with the progression and prognosis of gastric carcinomas.
